The Alaskan Front - Flashback
In a flashback to the Sino-American War, U.S. Marines Cooper Howard and Charlie Whiteknife are searching for a lost comrade (Raider 3) on an island in the Alaskan front against the People's Liberation Army of China. As they near a destroyed vertibird burning in the distance, the two assess the condition of their T-45 power armor suits, finding various malfunctions that limit their abilities. Cooper requests permission to abort their mission over his radio but is denied by his superiors. When Charlie asks about their orders, Cooper lies that they were told to head back and sends Charlie ahead while he stays there alone.

As Cooper looks over the wreckage of the vertibird, he hears snarling and growls, then gets a brief glimpse of a horned figure silhouetted in the flames. Cooper is then ambushed by three Chinese soldiers, with their weapons managing to damage his power armor further, resulting in the limbs locking up as he falls over unable to move. Approaching him, one of the soldiers, Wenjie, gloats about the T-45 power armor supposedly changing the face of warfare as she tells her comrades that their military leaders will likely send them to fight in power armor in the future as well. While the soldiers are distracted, however, Cooper notices the figure of a deathclaw coming up from behind, and the creature attacks and kills the Chinese soldiers. It leans in close to Cooper, terrifying him, but is prevented from acting further when it is drawn to sounds of a firefight in the distance. Soon after, Cooper's superiors radio in, saying that the enemy forces are retreating from the island and congratulating their troopers for pushing back the Reds. Cooper, left shocked at what he saw, weakly says that it wasn't American soldiers that stopped them, but something else.

Bottling factory
Soon after killing Xander Harkness with his super sledge at the Sunset Sarsaparilla factory, Maximus tries to recruit Thaddeus to help him cover it up before the Brotherhood finds out. They hear a vertibird arrive, with Maximus realizing that the Brotherhood had tracked them. Despite Thaddeus' reluctance, Maximus convinces him to help by pointing out the children he was protecting will be found by the Brotherhood, with the ghouls killed and the humans recruited, if they do not create a distraction.

Outside the factory, Maximus finds Knight Gnatius, who was assigned to pick them up. Gnatius tells him that Elder Quintus is furious that he went off alone with the Commonwealth emissary. He ignores Maximus' excuse and demands to know where Xander is. Thaddeus emerges, wearing Maximus' T-60 power armor to disguise himself as Xander, and clumsily tries to exit the factory's normal-sized door and board the vertibird, to Maximus' chagrin. They depart from the factory and fly past the skyline of New Vegas, heading back to Area 51.

NCR outpost
Lucy wakes up on a cot at the NCR outpost, brought there by the Ghoul after he saved her from crucifixion at the hands of Caesar's Legion, She notices she got an IV treatment of Buffout to help with her recovery as Captain Rodriguez and Ranger Biff introduce themselves as members of the New California Republic; Lucy questions if they mean they are all there is left, but Biff claims there are others residing elsewhere. He asks if Lucy has been to Shady Sands, as Rodriguez laments that their home was destroyed by "some asshole" who wanted to wipe out the first sign of civilization in the wasteland in a century. She offers Lucy an opportunity to join up with them, but the Vault dweller responds vaguely that she is looking for someone instead. When asked why, she remains vague but says she will bring them to justice, which Rodriguez accepts. The captain instead offers the remaining contents of their weapons cache, as they are packing up their camp and leaving as well. As the pair leave, Lucy tells the Ghoul she thought the Rangers were nice, but he reminds her that they likely would not be if they knew she was related to, and hunting down, the man responsible for destroying their home.

Vault 33
Inside Vault 33, the latest meeting of the Products of In-breeding Support Group is in full swing, with significantly more members than before. However, it becomes evident that most of them are only involved with Reg's group because he gets snacks and guaranteed water rations due to his arrangement with Betty. As Reg goes on another water collection, he crosses the overseer as she is supervising a dwindling crop field. Betty chastises him for wasting more water catering to his "snack club" that could be used to help the rest of the Vault. Reg is unfazed, reminding Betty she is dropping in popularity and had agreed to his terms for creating his support group, though he still meekly apologizes for his rude tone before hurrying away to get the water.

Mojave Wasteland
As Lucy and the Ghoul walk down a desert road, Lucy is shown to be visibly uncomfortable, including itchiness, erratic thoughts, heavy breathing, and general irritability. She asks if the Ghoul knows that feeling, which he affirms, revealing that she has become addicted to the Buffout used to rejuvenate her and is experiencing withdrawal symptoms. He claims that she has two options ahead of her: the first is to completely avoid more drugs and fight through the withdrawal symptoms, a process of a few days that will slow their travel, as Lucy sees they have reached the city limits of New Vegas with the Lucky 38 in sight. Her other option is what the Ghoul prefers, simply doing more drugs. The drug-addled Lucy talks herself into it and takes the Ghoul's offer of more Buffout, then resumes the walk into the city.

Area 51 - I
Maximus and "Xander" return to the flight deck of the Caswennan, where Dane lets Maximus know again that Quintus is furious with him. The scribe then notices the supposed paladin struggling to climb out of the vertibird in his power armor, and deduces right away something went wrong; Maximus confirms that he killed the real Xander, causing Dane to wryly respond that he has effectively ensured war with the Commonwealth Brotherhood is inevitable. Maximus tries to convince Dane that he has a plan to prevent the war, telling Dane to lead the chapter's aspirants to safety, as he intends to kill Quintus. Dane is left shocked, while Thaddeus continues to awkwardly navigate around in his power armor.

Los Angeles
The expedition from Vault 31 travels down a street in the urban ruins of Los Angeles, where they temporarily stop to raid the contents of a Blamco truck for food. Norm notices Claudia staring at the nearby bridge, which she says she remembers driving across on her way to her Vault-Tec job interview a week before being frozen and then waking up 200 years later in the wasteland. They are interrupted by another Vault 31 dweller, Ronnie McCurtry, who asks Norm about starting "phase two," boldly attempting to take the lead. Norm asks his name and then tries to dismiss him, saying he decides their next move. Ronnie, however, continues by revealing he used to be Bud Askins' personal assistant, collecting the lunch orders while Bud had discussions with the investors behind Bud's Three Vaults. He alludes to knowing some details about the true nature of Vaults 32 and 33, and mentions "Future Enterprise Ventures," the name Bud used for phase two of his experiment. An unnerved Norm steps back to allow Ronnie to lead the way, realizing he needs to tread carefully going forward.

Vault 32 - I
Inside Vault 32, Woody has been having difficulty navigating the unfamiliar layout of his new home. He accidentally wanders into Davey's quarters, and Davey empathizes with Woody's navigation struggles.

Meanwhile, Betty and Steph have a private meeting in the inter-vault space. Betty informs Steph that she checked on Vault 31 and found it empty, all of the occupants including Norm gone, and Bud Askins dead. She notices, though, that Steph is unfazed by the news. Betty tries to get her fellow overseer's help with sharing water between the Vaults, but Steph declines, saying that it would taint the "work" going on in her Vault. This confuses Betty, as Bud's death means the Three Vaults experiment has no reason to continue, especially since she is certain that Vault-Tec is gone and nobody is coming to help them, but Steph simply replies that they are doing what they have always done: surviving. Betty continues to push for a cooperative effort as Steph concedes to an exchange: she will "consider" giving access to 32's water supply only if Betty can retrieve a locked Keepsake Box and its contents formerly belonging to Hank. Betty questions what she wants from it, but Steph only tells her that her objective is "something personal."

Right then, Woody manages to stumble across the end of their conversation unnoticed after getting lost again. He overhears as Betty accuses Steph of essentially holding 33 hostage, which Steph does not directly deny. Betty remarks that she does not understand the reality of hardship living underground, but Steph alludes to having experienced hardships in her past which Betty, being a Solvang native, had not. Betty tries to beg Steph to help one more time, saying there won't be a Vault 33 anymore without getting water. Steph just shrugs her shoulders, saying the experiment is over and they make their own decisions now. A shocked Woody quickly retreats back into Vault 32 before Steph can notice him.

New Vegas Strip - I
On the outskirts of the New Vegas Strip, a jittery Lucy looks around while the Ghoul observes the entrance gate to the Strip in the distance. He notices that the place is suspiciously quiet and deserted; Lucy jokingly asks if he was expecting a "welcoming committee". Through his spyglass, the Ghoul spots a wrecked Securitron unit, then looks at a large group of ghouls wearing the tattered outfits of their former gang, the Kings, loitering around the entrance. He grimaces for unknown reasons and suggests they turn back and instead head around to the other entrance on the east end, going through Freeside, but Lucy protests that the half day's walk will take too long. She goes on ahead towards the ghoul pack, to the Ghoul's chagrin, though he admits to liking her newfound drug-fueled bravado.

As Lucy approaches the gate, a few of the ghoulified Kings notice and growl at her while she tries to intimidate them. When one of the ferals snarls loudly at her, she swiftly raises her lever-action rifle and shoots off his right foot. Lucy's initial surprise at easily resorting to violence only lasts for a moment before she gleefully opens fire on the rest of them. Two more charge at her from behind a car that the Ghoul and Dogmeat are leaning against, but she shoots both of them dead (the second as the Ghoul is preparing to stab him with his knife), before finishing off the ghoul whose foot she shot off. She then drops the rifle to pick up a lever-action shotgun, walks over to a nearby ghoul sitting in a wrecked car, and shoots him point-blank. Another ghoul takes Lucy by surprise and tackles her to the ground, but she manages to draw her pistol and shoots it in the head. She gloats to the Ghoul, though he is unnerved when she comments it's okay to kill them because "they're just ghouls." She grabs a radium rifle from her bag, kills two more feral ghouls, and then shoots a white-suited ghoul in the back of the head from a distance as he tries to stumble away. As Lucy relishes the violent spectacle, she enthusiastically asks the Ghoul for a cigarette, which the Ghoul denies. He looks around at the destroyed Securitrons, wondering what happened, while Lucy tries to steady her jittery gun hand amidst her drugged state as they get ready to walk into the Strip.

Area 51 - II
Thaddeus continues his poor attempt to remain discreet, impersonating Xander as he tries to navigate the Area 51 mess hall, unaware that he stands out like a sore thumb and everyone can hear him talking to himself through the helmet's speaker. As he gives up and attempts to leave, Elder McRae intercepts him, thinking that he's Xander, and asks for a private audience which he is forced to accept. Elsewhere, Dane follows Maximus' plan as they move the young aspirants into a hiding place, telling them not to come out "until the shooting stops." In the meeting room, Maximus meets with Quintus alone, where the elder cleric figures out something went wrong during his adventure. Maximus confirms that he killed Xander and, as he draws his laser pistol, the unfazed Quintus remarks that Maximus intends to kill him too.

Meanwhile, Thaddeus is forced to listen as Elder McRae tries to strike a deal to protect her chapter from the Commonwealth's wrath for supporting Quintus. However, they are interrupted by the Coronado elder barging in and trying to make his own deal, offering to assassinate Quintus in exchange for 300 fusion cores; McRae tries to preempt him by offering to carry out the deed for nothing but the recognition of Elder Maxson. The stress causes Thaddeus to have a panic attack in his suit, to the elders' confusion. McRae helpfully tries to lift his visor, but Thaddeus moves back from her. Meanwhile, in the central warehouse, Dane finds the cold fusion device unguarded and plans to steal it.

Noticing Maximus hesitating to attack, Quintus moves toward him. He chastises Maximus that his actions will only doom the world to remain in its current anarchic state, while only Quintus himself could lead it to change for the better. Maximus struggles to go through with killing him, saying he never planned for any of the events he experienced but still felt he had to do the "right" thing. He admits to Quintus that Xander was trying to murder children, leaving the elder cleric perturbed and seemingly somewhat sympathetic. This sympathy instantly evaporates however when Maximus reveals that said children were ghouls, prompting Quintus to angrily rant that he went against the Codex's law to exterminate "abominations." He spitefully rebukes Maximus, saying he should have never shown mercy to him, then retrieves a hidden 10mm pistol and starts a firefight that forces Maximus into cover.

Just before Thaddeus' identity can be exposed, the Grand Canyon elder bursts into the cafeteria, revealing to everyone that the cold fusion device has been stolen, and accuses both of the other elders of being the thieves. The Coronado elder tries to radio his soldiers to take action, but McRae stabs him through the cheek from behind, briefly surprising the Grand Canyon elder before he puts a bullet in her shoulder. This ignites a fight that quickly engulfs the rest of the Brotherhood chapters; Thaddeus takes the opportunity to flee.

Maximus is forced to give up his attack and escape the meeting room while Quintus hurls accusations of "heretic" at him and shoots until he runs out of bullets. The ex-knight traps his former leader inside and sees the ongoing battle nearby, just as Dane runs in to find him. Maximus apologizes for not killing Quintus but Dane consoles him, saying he should not be ashamed of refusing to commit murder. The scribe presses the stolen cold fusion relic into his hands, telling him to take it and run far away from the Brotherhood; they emphasize their belief in Maximus as the only Brotherhood member that's "not a total asshole." As Thaddeus runs past, Maximus questions what Dane intends to do, with Dane responding that they will survive like they always have. Maximus follows Thaddeus as they run out into the Mojave while the Brotherhood airships begin firing on each other, resulting in one of them being destroyed and the fiery remains crashing into the dunes.

Vault 32 - II
Steph hosts a meeting with Woody in her quarters she shares with Chet, who does laundry nearby while taking care of Chet Jr.. Woody says he came to report a violation of the Vault handbook and exposes that he had overheard Steph's meeting with Betty in the inter-vault space, something done against the rules as the area is prohibited outside of the Triennial Trade. Steph thanks him for reporting it with a forced smile, saying she will escalate the matter immediately. This confuses Woody as he wonders who she, as the overseer, would escalate it to, but Steph replies that it would be herself. Woody tries to ask what she was doing meeting with Betty, but Steph coldly dismisses him and tells him to address her as Overseer Harper.

During their conversation, Chet finds a false bottom to a drawer in Steph's dresser, which he discreetly looks into and uncovers an old purse. He takes out an ID card from the sleeve, revealing Steph's pre-War birthdate in 2045 and that she was originally from the annexed territory of Canada, which shocks him and reminds him of the secrets he and Norm learned about the Vaults before. Wanting to avoid being caught, he hurriedly puts it back as the meeting ends, and makes small talk with Woody. Just as Woody exits the room, he turns back and asks Steph about what she meant by "[the] experiment," but she again dismisses him by calling it "overseer stuff" and shuts the door. The frustrated Steph has Chet worried as well as frightened by her potentially finding out about his discovery.

New Vegas Strip - II
As nightfall comes, Lucy and the Ghoul walk through the deserted streets of the New Vegas Strip. Lucy vocalizes what is clearly off about the place: there are no people anywhere to be seen. The Ghoul emphasizes that they should go around to Freeside again but Lucy opts to loudly shout out for Hank. The Ghoul walks ahead, following Dogmeat as he looks up at the lit rooms of the Lucky 38. He then picks up on the unusual presence of radiation on the Strip thanks to the Geiger counter on Lucy's Pip-Boy going off. Lucy herself notices and begins playing with a large, cracked eggshell, making a joke about massive chickens. When she shows it to him, all of the blood drains from the Ghoul's face as he recognizes her find as a deathclaw egg, right as they both hear loud roaring coming from nearby.

Lucy's Buffout-charged bravado has her believe they can take on whatever the threat is, but the Ghoul is stricken with fear. They look towards the glass door entrance of Gomorrah and a monstrous shadowy figure moving inside. Lucy notices the uncharacteristic terror on the Ghoul's face as her cheer falls away and she becomes more aware of the danger. Long, sharp claws poke through the door gap before a massive deathclaw rips the casino's doors off and advances out onto the streets. It notices the pair and roars as it stands up to its full height, causing Lucy to mutter a nervous "okey-dokey."
