Both the Ghoul and Moldaver recognize the MacLean name, which means Rose was operating in surface power structures long before anyone in Vault 33 knew the outside world existed.
How This Theory Works
Rose MacLean opened Vault 32 from the outside, and the most uncomfortable reading of that fact is not that she was coerced or deceived -- it is that she chose to do it with full knowledge of what was inside, because the people inside knew something she needed suppressed or contained. That is the interior truth the theory approaches without committing to: Rose did not breach Vault 32 as an agent of surface factions working against the vaults. She may have done it as an agent of the vault system itself, protecting a secret the residents had already identified in blood on the wall.
The door logs Norm finds do not leave ambiguity about whose credentials authorized the entry. Rose's Pip-Boy opened Vault 32. The residents were already dead or dying, some by suicide, some by violence, the last Pip-Boy reading two years old. The inscription 'WE KNOW THE TRUTH' and the looping television program about mice in an overpopulated environment suggest the vault had already fractured from within before any outsider arrived. Rose may have opened the door to a vault that was already gone, or she opened it knowing what it would trigger, or she opened it to ensure no one who knew the truth made it out. None of those possibilities are confirmed. All of them implicate her in a way that goes beyond faction loyalty and into something more intimate and more deliberate.
The Ghoul's reaction to the surname MacLean is the sharpest pressure point. He was not indifferent. His surprise read like recognition, the kind that carries history behind it. Moldaver also knew the MacLean name before Lucy surfaced. A pre-war ghoul and the leader of an armed surface faction both carry prior knowledge of this family. Rose is the most plausible connection between them. If she has been operating on the surface for two years with a Pip-Boy and access to vault infrastructure, she is not a missing mother. She is a participant in whatever operation both figures are running, and she has been one long enough that her name lands with weight on people who have survived a century of weight.
